HOW THE MODEL WORKS

Subspecialists work within the same coordinated structure as primary care, sharing patients and a single quality framework. Vent and trach populations are reviewed in real time, respiratory and renal cases are addressed during active clinical decision-making, and Medical Directors bring continuity across the whole team. In high-acuity facilities where this integration is in place, avoidable transfers have declined and clinical stability has improved.
